Job summary

Providence at Home with Compassus seeks a Hospice RN for part-time, day shifts with Saturday and Sunday 10-hour shifts. You will assess patients in their homes, coordinate with the IDG, and deliver compassionate hospice care across the care continuum.

You will develop individualized care plans, educate families, and manage symptoms while following the nursing process. On-call duties and accurate documentation are required.

Qualifications

• Active RN license in the state of employment.
• Hospice nursing experience preferred.
• Proficiency in patient and family education and care planning.

Responsibilities

• Completes patient and family assessments to determine hospice needs.
• Develops and initiates Plan of Care with the IDG and revises as needed.
• Administers medications and treatments per the Plan of Care.
• Participates in on-call duties and maintains accurate documentation.
