Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, person-centered care in patients' homes. You will assess needs, plan with the Interdisciplinary Group (IDG), and provide professional nursing care grounded in the nursing process.

In this role you’ll support patients and families with dignity and comfort at end-of-life, while collaborating with the IDG and maintaining documentation and education for ongoing care.

Qualifications

• Associate or Bachelor’s degree in Nursing required.
• Current, unencumbered RN license in State of employment.
• BLS certification required or within 30 days of hire.

Responsibilities

• Assess patient and family needs and develop Plan of Care with IDG.
• Provide professional nursing care in home environment per nursing process.
• Administer medications and treatments as prescribed by IDG Plan of Care.
• Participate in on-call duties and maintain timely documentation.
• Ensure accurate clinical notes and ongoing patient education.
