Job summary

Hartford HealthCare at Home is seeking a Primary Care Nurse to provide skilled nursing care to hospice clients in their homes across Connecticut. The weekend shift covers 32 hours with pay for 36, continuing the organization's mission to deliver high-quality, patient-centered care.

The role requires CT nursing licensure, 1 year of nursing experience (home care or medical/surgical preferred), and an associate degree or diploma in nursing.

Qualifications

• Education: Associate’s degree in nursing required.
• NLN-approved BSN program preferred.

Responsibilities

• Provides skilled nursing care to hospice clients in their homes or community settings.
• Reviews referrals and initiates contact with client and family.
• Performs nursing assessments based on referrals and clinical observations.
• Develops care plans in collaboration with client and disciplines; documents goals and resources.
• Facilitates bereavement follow-up and timely documentation.
• Monitors patient status and adjusts care plans as conditions change.
• Coordinates with physicians and IDT; guides LPNs and aides.
• Educates clients and families on care, signs, and symptoms.
