Job summary

Hartford HealthCare at Home in Hartford, CT is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in their homes. This role involves collaborating with the interdisciplinary team to ensure high-quality, compassionate end-of-life care.

You will perform assessments, develop care plans, teach families, coordinate with physicians, and supervise LPNs and home care aides. A CT nursing license and one year of nursing experience are required; BSN preferred.

Qualifications

• Graduate of an Associate’s or diploma nursing program.
• Active Connecticut nursing license.
• One year nursing experience or one year nursing experience in a specialty.
• BSN preferred; home health or hospice experience desirable.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts full nursing assessment based on referral data and observations.
• Develops care plans collaboratively with client and interdisciplinary team.
• Delivers skilled nursing care and manages medications and DME approvals.
• Maintains client records according to agency standards.
• Educates patients and families on care, signs, symptoms, and home care needs.
• Directs LPNs and Home Care Aides; supervises plan of care.
• Coordinates care with physicians and community resources.
