Job summary

You will travel to patient homes, perform urgent assessments, manage symptoms, and document in EMR while supporting families during weekends and crises.

Qualifications

• Active RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver’s license and ability to drive to patient locations ~20% of the time.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Perform urgent assessments, manage symptoms and pain, and provide end-of-life support.
• Support and guide patients and families during weekends and crises, ensuring they are informed and cared for.
