Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Respond to after-hours needs by performing urgent assessments and managing symptoms.
• Support patients and families during evenings, nights, weekends, and crises.
• Coordinate care with the interdisciplinary team and maintain accurate EMR documentation.
