Job summary

Graham Healthcare Group is seeking a Weekend Registered Nurse (RN) to deliver end-of-life hospice care in patients’ homes around Brighton, MI. The role includes driving to patient locations, collaborating with an interdisciplinary team, and documenting care in the EMR system.

Qualifications

• Active RN license in the state you work (MI).
• Two years of RN experience; hospice experience preferred.
• Ability to drive to patient locations ~20% of time.
• Strong clinical judgment and ability to coordinate care.

Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes.
• Respond to urgent needs, manage symptoms and comfort, and document care in EMR.
• Support families and patients during weekends and crises with clear communication.
• Travel to patient homes within the assigned territory and work independently.
