Job summary

Graham Healthcare Group is seeking a Weekend Registered Nurse (RN) in Luzerne County, PA to provide compassionate hospice care in patients' homes. The role covers weekend shifts, with a focus on comfort, dignity, and family support.

You will collaborate with an interdisciplinary team, manage symptoms, and document care in the EMR. A valid RN license in Pennsylvania and driving to patient locations are required.

Qualifications

• Active Registered Nurse license in the state you work.
• Two years of RN experience; hospice experience preferred.
• A valid driver’s license with ability to travel to patient homes.

Responsibilities

• Deliver patient-centered hospice care in a home setting as part of an interdisciplinary team.
• Perform urgent assessments, manage symptoms and pain, and provide end-of-life support.
• Support families during weekends and crisis situations, ensuring clear communication and reassurance.
• Coordinate care with strong clinical judgment and maintain EMR documentation.
• Work independently but collaborate with the team as needed.
• Travel to patient homes within the assigned territory.
