Job summary

Graham Healthcare Group is seeking a Weekend Hospice Registered Nurse (RN) in Plymouth, Pennsylvania to provide end-of-life care in patients’ homes. This staff position offers 40 hours per week, with an 8-hour shift, primarily serving weekends in the Wilkes-Barre area with a $10,000 first-year bonus separate from base compensation.

You’ll collaborate with an interdisciplinary team to deliver compassionate, patient-centered care, travel to patients' homes, and maintain accurate EMR documentation

Qualifications

Active RN license in Pennsylvania (or eligible)
2+ years RN experience, hospice experience preferred
Valid driver’s license and ability to travel to patient homes approx. 20% of time

Responsibilities

Deliver compassionate, patient-centered hospice care with an interdisciplinary team
Perform urgent assessments, manage symptoms and pain, provide end-of-life support
Support families during weekends and crisis moments, ensure informed care
Coordinate care with accurate EMR documentation and clinical judgment
Work independently in the field, traveling to patient homes within territory
