Qualifications

• Active RN license in the state you work.
• One year of RN experience in home health or clinical setting.
• Valid driver's license with ~20% travel to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, with interdisciplinary support.
• Perform patient assessments, medication management, and care planning.
• Educate families on disease management and treatment plans.
• Document thoroughly in the EMR and work independently in the field.
• Travel to patient homes within assigned territory.
