Responsibilities

• Deliver compassionate, patient-centered hospice care in patients' homes, collaborating with an interdisciplinary team
• Respond to patient needs with urgent assessments, symptom and pain management, and end-of-life support
• Support and guide patients and families during weekends and crisis situations
• Coordinate care with the team and maintain accurate, timely EMR documentation
• Work independently, but collaborate when needed and travel to patient homes within territory
