Job summary

Hartford HealthCare at Home is seeking a Weekend Hospice RN to provide skilled nursing care to hospice clients in home and community settings. You will perform comprehensive nursing assessments, develop and adjust care plans, and collaborate with the interdisciplinary team to ensure high-quality, cost-effective care.

Qualifications

• Graduate of an Associate’s or diploma nursing program.
• CT licensure to practice nursing in CT.
• One year nursing experience or one year in a specialty.

Responsibilities

• Provide skilled nursing care to hospice clients in home/community settings.
• Conduct comprehensive nursing assessments and develop care plans with client/family.
• Coordinate care with interdisciplinary team and other providers.
• Educate patients and families on care, signs, and disease management.
• Document client records and bereavement follow-up.
• Refer and collaborate with community resources as needed.
