Job summary

Hartford HealthCare at Home is seeking a skilled Hospice Registered Nurse for weekend coverage in the Hartford area. The role provides in-home nursing care to hospice clients, with a 3/4 shift pay structure and a day shift schedule.

The RN will perform comprehensive assessments, develop and implement a care plan, coordinate with the interdisciplinary team, and educate clients and families on end-of-life care, medication management, and available resources.

Qualifications

• Education: Graduate of an Associate’s or diploma program in nursing.
• Experience: One year nursing experience or one year nursing experience in a specialty.
• Licensure: License to practice in the State of Connecticut.
• Preferred: Education: NLN-approved BSN program; Experience in medical/surgery or home care.

Responsibilities

• Reviews referrals and initiates contact with client and family.
• Conducts comprehensive nursing assessments based on referral data and patient status.
• Develops a care plan in collaboration with client and interdisciplinary team.
• Manages medications, including non-formulary requests, per payer guidelines.
• Maintains client records and documents care activities.
• Performs follow-up visits to assess progress and adjust care as needed.
• Educates patients and families on signs, symptoms, and home care practices.
• Coordinates care with physicians and other providers.
• Directs LPNs and Home Care Aides in carrying out the plan of care.
