Amedisys, part of the Optum family, is seeking a Physical Therapist to deliver skilled PT services to patients in their homes, promoting mobility, independence and safety. You will work with patients, caregivers and an interdisciplinary team to support recovery and prevent hospitalizations in the home setting.

Requirements include a state PT license, CAPTE-accredited degree, CPR certification, and at least 1 year of home health experience (mentoring if less).

Competenze

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• At least 1 year of home health care PT experience; if <1 year, mentoring/approval required.
• Current CPR certification.

Mansioni

• Help patients maintain, restore, and improve movement, activity, and function.
• Provide interventions, conduct examinations at least every 30 days, adjust plans, and perform discharge planning.
• Collaborate with patients, caregivers and an interdisciplinary care team to support recovery and prevent hospitalizations.
• Other duties as assigned.
