Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to deliver skilled services in patients' homes, promoting mobility, restoring function, and enhancing independence. You will work with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalization, with reexaminations every 30 days and discharge planning as part of the care plan.

This role requires CAPTE-accredited education, a state PT license, CPR certification, and a valid driver’s

Qualifications

CAPTE-accredited physical therapy program completion.
Current license to practice physical therapy in assigned state.
Current CPR certification.
1+ year of related PT experience in home health care preferred.
Must have driver's license and reliable transportation.
If under 1 year, regional clinical leaders must approve with mentoring or residency.

Responsibilities

Helping patients maintain, restore and improve movement, activity and function.
Provides interventions, conducts reexaminations (every 30 days, at minimum), modifies interventions as necessary, and performs discharge planning.
