Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to provide skilled services in patients' homes. You will promote mobility, restore function, and improve safety with an interdisciplinary care team.

Responsibilities include patient movement interventions, periodic reexaminations, discharge planning, and other duties as assigned. Requires CAPTE-accredited degree, valid state license, CPR, and 1+ year home health experience.

Qualifications

• Graduate from a CAPTE-accredited PT program.
• Current license to practice PT in the assigned state.
• Current CPR certification.
• 1+ year PT experience in home health; mentors if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Provide interventions, conduct reexaminations at least every 30 days, modify plans, and perform discharge planning.
• Other duties as assigned.
