Senior Care Solutions in Boca Raton, FL is seeking a Baylor Intake Coordinator to manage weekend referrals and admissions for home health patients. You will coordinate with physicians, hospitals, and case managers to ensure timely Start of Care visits and accurate documentation in the EMR.

The ideal candidate has prior intake or healthcare administrative experience, strong organizational skills, and proficiency with Microsoft Office and EMR systems.

Qualifications

• Previous intake, scheduling, or healthcare administrative experience.
• 1+ years of Home health/Medicare experience.
• Strong organizational and multitasking skills.
• Excellent verbal and written communication.
• Proficiency with Microsoft Office; EMR experience preferred (WellSky/Kinnser experience is a plus).
• Ability to work independently while managing multiple priorities.
• Detail-oriented with strong problem-solving skills.

Responsibilities

• Process and coordinate new home health referrals and admissions.
• Verify patient information and ensure all required documentation is complete.
• Communicate with physicians, hospitals, case managers, and referral sources.
• Coordinate with clinical staff to facilitate timely Start of Care visits.
• Maintain accurate patient records in the EMR system.
• Monitor referral status and ensure compliance with Medicare and agency guidelines.
• Provide exceptional customer service to patients, families, and referral partners.
• Collaborate with the weekday intake team to ensure seamless continuity of care.
