Job summary

Choice Health At Home in Boca Raton, FL is seeking an Intake Coordinator to manage initial patient referrals, verify payor sources, and coordinate with nursing staff to ensure a seamless start of care.

Qualifications

• Minimum one year of experience in referral intake.
• Experience with Medicare and Medicaid is highly desirable.
• Medical field background and familiarity with medical terminology.
• Excellent computer and customer service skills.
• LVN/LPN preferred.

Responsibilities

• Receives and appropriately documents new referrals in a timely manner.
• Verifies each patient's payor source and maintains and tracks insurance authorizations and initial certifications.
• Participates in coordinating care with Nursing Supervisors, patients, and the interdisciplinary group.
• Provides effective communication to patients, staff members, other health care professionals and referral sources.
• Responds to Agency and patient needs in a professional and creative manner.
• Demonstrates commitment, professional growth and competency.
• Participates in Agency-sponsored in-service training.
• Participates in the Agency's QAPI program.
• Promotes the Agency philosophy and administrative policies to ensure quality of care.
