Job summary

RVNAhealth is seeking an Extended Hours Coordinator to support our Home Health and Hospice services during weekend and holiday hours. This part-time, remote-from-home role requires residing within commuting distance of Ridgefield, CT, with onsite training as needed.

You will coordinate referrals, schedule patient visits, answer calls, verify insurance, and maintain accurate documentation while aligning with regulatory guidelines. Weekend availability and four holidays per year are required.

Qualifications

• Minimum of 2 years healthcare experience
• Familiarity with medical terminology
• Must reside within commuting distance of Ridgefield, CT 06877
• Able to report onsite for training, meetings, or other business needs

Responsibilities

• Coordinate after-hour and weekend home health and hospice referrals.
• Schedule patient visits based on physician orders and update schedules.
• Answer calls from patients, families, and referral sources with timely support.
• Communicate with the Extended Hours Triage RN, field clinicians, and referrals.
• Enter and manage patient intake information, insurance verification, and authorizations.
• Ensure compliance with regulatory guidelines and proper documentation.
• Support weekend office functions including intake processing and clinical team communication.
• Escalate issues to clinical and admin leaders as needed.
• Maintain understanding of payer requirements (Medicare, Insurance, Title 19).
• Uphold agency policies and contribute to quality improvement.
