Job summary

RVNAhealth in New Milford, CT is seeking a Home Health Coordinator to schedule and coordinate patient visits across disciplines, supporting clinician productivity and patient satisfaction.

You will manage SOC/ROC visits, assign a primary case manager, monitor timeliness, and ensure compliance with Medicare guidelines. This full-time, on-site role offers a Monday–Friday schedule and comprehensive benefits.

Qualifications

• 2+ years of healthcare experience required, with medical terminology knowledge.
• Proficiency with EHR and scheduling systems.
• Exceptional customer service skills, including phone demeanor and relationship-building in a fast-paced environment.
• Bachelor's degree or equivalent healthcare experience preferred.

Responsibilities

• Schedule and maintain accurate patient visit calendars across disciplines.
• Schedule SOC and ROC visits in the scheduling system.
• Assign and maintain a primary case manager for each patient.
• Coordinate clinician coverage for vacations and sick time.
• Communicate visit schedules and changes with patients and families.
• Ensure timely visits to support billing, compliance, and quality outcomes.
• Reconcile visits with physician orders to ensure regulatory adherence.
• Monitor recertification timelines and provide weekly reminders to clinical staff.
• Identify scheduling gaps and risks and collaborate with leadership to resolve them.
• Assist in balancing coverage across service territories.
