Job summary

RVNAhealth is seeking a Home Health Coordinator to schedule and coordinate patient visits across disciplines from our New Milford, CT office. The role supports clinician productivity, episode management, and patient satisfaction while collaborating with clinical teams, patients, and families.

Responsibilities include maintaining accurate schedules, SOC/ROC visits, ensuring regulatory compliance, and using scheduling systems to optimize care delivery in a fast-paced home health environment.

Qualifications

• 2+ years of healthcare experience.
• Bachelor's degree or licensed/experienced health care personnel.
• Minimum one year of community health experience preferred with knowledge of home health or hospice regulations.
• Knowledge of Medicare guidelines, episode management, and LUPA implications preferred.
• Strong organizational, multitasking, and problem-solving skills.
• Proficiency with EHR and scheduling systems.
• Exceptional customer service skills including phone manner and ability to build relationships.

Responsibilities

• Maintain accurate patient schedules and assign visits across disciplines.
• Schedule SOC and ROC visits in the scheduling system.
• Assign and maintain a primary case manager per patient.
• Coordinate clinician coverage for absences.
• Communicate visit schedules with patients and families.
