Responsibilities

• Review and assess Home Health and Hospice referrals for appropriateness and urgency.
• Obtain and verify referral documentation and insurance information.
• Coordinate with on-call nurses, case managers, and leadership for weekend admissions.
• Enter accurate referral and patient data into the EMR.
• Communicate professionally with referral sources, patients, families, and staff.
• Ensure HIPAA, CMS, and state regulations compliance.
• Maintain clear documentation of intake activities.
