AdventHealth in Orlando, FL is seeking an experienced RN Case Manager to optimize discharge planning, coordinate post‑acute care, and facilitate multidisciplinary rounds. You will assess patient needs, educate families, and collaborate with payors to secure timely services.

The role emphasizes leadership, data analysis, and effective communication across diverse teams in a dynamic hospital setting. Requires RN licensure, 2+ years of hospital experience, and CMS discharge knowledge.

Qualifications

• Associates of Nursing is required.
• Bachelor's degree in nursing preferred.
• Active RN license in Florida is required.
• Minimum 2+ years of medical/hospital nursing experience with care management/utilization focus.

Responsibilities

• Actively participates in multidisciplinary rounds to review patient status and discharge plans.
• Educates patients and families on illness impacts and resources; advocates for patient/family decisions.
• Assesses readmitted patients for reasons and informs care team.
• Organizes and facilitates patient and family care conferences with the team.
• Documents discharge planning evaluations, barriers, and payor needs for authorization.
• Assesses wholistic discharge planning needs across inpatient/ED settings to avoid delays.
• Reviews medical records and integrates clinical and social factors into the discharge plan.
• Develops discharge plans with contingency planning and post-acute care arrangements.
• Utilizes technology to communicate with post-acute services for continuity of care.
