Qualifikationen

• High school diploma or GED required.
• Must be at least 18 years old.
• Valid driver’s license in good standing.
• Must successfully complete all background checks.
• Must successfully complete health screening (TB) and any state-specific requirements.

Aufgaben

• Assist individuals with goal completion as documented in the individualized support plan.
• Provide assistance with Activities of Daily Living (ADLs)—bathing, grooming, toileting, feeding, meals.
• Safely transport individuals in company or personal vehicles.
• Accompany individuals to community integration activities.
• Complete all required documentation in the electronic health record (EHR) timely.
• Engage with individuals in home and community and advocate for their rights.
