Job summary

Qualifications

Responsibilities

• Assist individuals with goal completion per their support plan.
• Help with ADLs such as bathing, grooming, toileting, and meals.
• Safely transport individuals to activities and appointments.
• Document activities in the electronic health record (EHR).
• Respect rights, maintain confidentiality, and follow HIPAA regulations.
• Maintain professional boundaries and report abuse or neglect.
