Job summary

Highmark Inc. is seeking a Wellness Coach to manage a panel of members with varying health needs. You will assess health management requirements, develop personalized care plans, and coordinate with a multidisciplinary team to drive better health outcomes.

You will conduct outreach via phone, digital or virtual channels, monitor progress, and document all activities in compliance with regulatory standards. Strong communication and organizational skills are essential.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• 3 years as a Registered Dietician OR Exercise Physiologist OR Wellness/Lifestyle Coach in a wellness-focused environment

Responsibilities

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Maintain oversight over specified panel of members by performing ongoing assessment of members’ health management needs and triaging to appropriate resources.
• Create care plans addressing identified needs, remove barriers to care, identify resources, and conduct activities to improve health outcomes.
• Demonstrate clinical outcomes and cost savings based on wellness interventions and support members across age groups and regions.
• Ensure all activities are documented and compliant with requirements; participate in workgroups to improve wellness coaching services.
• Other duties as assigned or requested.
