Job summary

Highmark Inc. is seeking a qualified Wellness Coach to oversee a panel of members with varying health needs. You will assess health management needs, develop personalized care plans, and coordinate with a multidisciplinary team to drive behavior change and improve outcomes.

You will engage members remotely or telephonically, monitor progress, and ensure documentation and compliance with applicable standards and regulations. A master's degree and clinical experience are required.

Qualifications

• Master’s degree or equivalent in nutrition/health education or related field.
• At least 3 years as RD, Exercise Physiologist, or Wellness Coach.
• Preferred certifications: CDE, CHWC, Tobacco Cessation.

Responsibilities

• Develop personalized care plans for members across health statuses.
• Coordinate with multidisciplinary teams to refer to resources.
• Track outcomes and ensure compliance with program standards.
