Job summary

Mercy is seeking a Nurse Manager for the Medical Management & Transition Team to provide operational, clinical, and strategic leadership for virtual-based transitional care programs. You will oversee daily operations, quality outcomes, staff performance, and regulatory compliance to support patients moving through the care continuum.

The role emphasizes safe, patient-centered care, interdisciplinary collaboration, timely follow-up, and population health initiatives.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing in the applicable state of practice.
• Four (4) years of experience in clinical specialty or related area.
• CPR and NRP certifications.
• Medication and math competency test required before assuming medication administration responsibilities.
• BSN or completion within next 2 years.
• BS/MS preferred.
• At least 2 years of supervisory or management experience preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for Mercy’s virtual-based transitional care medical management and Mercy Lifestyle and Wellness programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance of services supporting patients transitioning across the care continuum.
• Ensure safe, effective, and patient-centered care delivery; foster interdisciplinary collaboration; ensure timely follow-up and care coordination; advance Mercy’s mission through population health strategies.
• Lead and develop teams to optimize care coordination, outcomes, and patient experience.
