Job summary

Mercy Nurse Manager for the Medical Management & Transition Team leads virtual-based transitional care programs, directing operations and ensuring quality and regulatory compliance. Responsibilities include optimizing patient care delivery and fostering cross-team collaboration to support the care continuum.

Qualifications

• Graduate of an approved school of nursing; BSN or complete in the next 2 years.
• Current licensure by the State Board of Nursing.
• 4 years of experience in clinical specialty or related area with management, supervision, teaching, performance improvement, and budget.
• CPR NRP certifications.
• Medication and math competency test for licensed coworkers who provide direct patient care.
• BS/MS preferred; 2 years supervisory/management experience preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Ensure safe, effective, patient-centered care delivery and interdisciplinary collaboration.
• Ensure timely follow-up and care coordination across the care continuum.
• Advance Mercy’s mission through high-quality virtual care management and population health strategies.
