Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, focusing on high-quality, virtual-based transitional care. You will guide daily operations, ensure regulatory compliance, and drive staff performance and patient safety across ED-to-Home and outpatient coordination.

The role emphasizes clinical leadership within a collaborative, technology-enabled care model and aims to improve outcomes while supporting Mercy’s population health goals.

Qualifications

• Education: Graduate of an approved nursing school; BSN or current enrollment for completion within 2 years.
• Licensure: Current nursing license in the applicable state.
• Experience: Four (4) years in a clinical specialty or related area with management, supervision, teaching, and budgeting.
• Certifications: CPR and NRP.
• Medication and math competency testing required for direct patient care.
• BS/MS preferred and supervisory/management experience preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Coordinate ED-to-Home, accelerated outpatient management, and chronic condition follow-up.
• Foster interdisciplinary collaboration and safe, patient-centered care delivery.
• Advance population health strategies through Mercy's mission.
