Responsibilities

• Administers intake capturing information on physical ability, service information, trauma exposure, and mental health history.
• Assists veterans with developing IRPs and goals.
• Refer veterans to service providers and facilitate meetings.
• Link veterans to providers by in-person or meeting facilitation.
• Provide trainings to community and Veterans organizations.
• Represent teams at public forums and interagency meetings.
• Advocate for client rights with providers and agencies.
• Attend trainings to maintain knowledge of veterans' issues.
