Job summary

Qualifications

• High School Diploma or equivalent.
• Valid Driver’s License.
• Experience in case management, social services, behavioral health, veteran services, or community outreach.
• Strong written and verbal communication skills.
• Ability to manage caseloads, prioritize work, and meet deadlines.
• Knowledge of military culture and veteran-related issues.
• Experience working with mental health, PTSD, TBI, suicide prevention, homelessness, or substance abuse programs.
• Ability to maintain confidentiality and follow HIPAA regulations.
• Comfortable with public speaking, after-hours events, and on-call responsibilities.
• Ability to work collaboratively with courts, government agencies, and community organizations.

Responsibilities

• Conduct veteran intake assessments and case management.
• Develop and monitor Individual Recovery Plans (IRPs).
• Connect veterans with healthcare, housing, behavioral health, employment, and community resources.
• Advocate for veterans with agencies, courts, employers, landlords, and service providers.
• Coordinate suicide prevention, crisis intervention, and substance abuse services.
• Build relationships with VA, federal, state, municipal, and community organizations.
• Provide outreach, training, and presentations on veteran programs.
• Maintain HIPAA-compliant documentation.
• Assist Veteran Service Officers and community partners in supporting veterans.
