Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Community Outreach Case Manager to support the SSG Fox Suicide Prevention program in VA. This full-time role requires building partnerships, identifying individuals in need of services, and coordinating community-based suicide prevention efforts, often with evening and weekend hours.

The position reports to the Program Director and involves travel, case management, and collaboration with VA and community providers to improve referral

Qualifications

• Bachelor’s degree in Human Services or related field required.
• 3–5 years outreach experience with Veterans or mental health/substance use/IDD concerns.
• Strong communication and resource navigation skills.

Responsibilities

• Coordinate outreach for the Veterans Program and VA partnerships.
• Screen participants for eligibility using VA forms and verify information.
• Provide case management services and wraparound support.
• Maintain client records and logs; travel to sites as needed.
