Nation’s Finest, a Veterans’ services nonprofit with over 50 years of history, seeks a Case Manager to support participants in achieving stability and self-sufficiency. The role focuses on client-centered case management, housing navigation, and employment support within a collaborative, trauma-informed environment.

Responsibilities include outreach, service planning, documentation, and coordination of benefits and resources across multiple sites to meet individual goals.

Qualifications

• Bachelor’s degree strongly preferred.
• Minimum 1 year of experience in case management or related social services.
• Knowledge of housing programs, employment resources, and crisis intervention strategies.
• Strong interpersonal, communication, and organizational skills.

Responsabilités

• Conduct client screenings, intakes and assessments to determine eligibility, needs, and goals for services.
• Develop, implement, and monitor Service Plans in collaboration with clients.
• Provide strengths-based case management including advocacy, goal setting, crisis intervention, and progress monitoring.
• Coordinate services including financial assistance, income and health benefits, and health care navigation.
• Educate clients on program expectations and support adherence to rules and policies.
• Conduct outreach in the community to engage individuals and connect them with resources.
• Build and maintain relationships with landlords, housing providers, and community agencies.
