Job summary

U.S. VETS, the nation’s largest nonprofit dedicated to ending veteran homelessness, seeks a Case Manager II to provide comprehensive support to veterans overcoming housing instability, trauma, mental health challenges, and other barriers to long-term stability.

You will perform trauma-informed, strengths-based case management, coordinate services with VA, landlords, and community providers, and conduct home visits to ensure the health and welfare of clients.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, or related field.
• At least two years of work experience in case management or other roles in Social Services.
• Ability to pass background checks if assigned to programs involving incarcerated or recently released clients.
• Strong communication, documentation, and organizational skills.
• Proficiency in Microsoft Office and electronic records systems.
• Valid driver’s license with clean driving record and ability to complete driver training.

Responsibilities

• Provide trauma-informed, strengths-based case management to veterans.
• Coordinate and monitor services with VA, landlords, employers, treatment providers, and community resources.
• Conduct regular one-on-one meetings and home visits.
• Transport clients to appointments and other meetings as needed.
• Facilitate life skills groups tailored to veteran populations.
• Assist with crisis intervention and after-hours support as needed.
• Document services and incident reports in electronic client record systems.
• Support reporting, audits, and file reviews with supervisor.
