Job summary

Nation’s Finest is a veterans’ services nonprofit providing housing, health and employment support across multiple sites in the United States. The Case Manager will support participants in achieving stability and self-sufficiency through client-centered services, outreach, housing navigation, and employment assistance.

Qualifications

• Minimum 1 year experience in case management or related field.
• Bachelor's degree in social work or related field preferred.
• Knowledge of housing programs, social services, employment resources, and crisis intervention strategies.
• Proven ability to manage administrative tasks across multiple funding streams and reporting standards.

Responsibilities

• Conduct client screenings, intakes and assessments to determine eligibility, needs, and goals for services.
• Develop, implement, and monitor Service Plans with clients.
• Provide strengths-based case management including advocacy, goal setting, crisis intervention, and progress monitoring.
• Coordinate services with staff and external partners to ensure holistic, integrated care.
