Job summary

Volunteers of America Chesapeake & Carolinas seeks a Community Outreach Case Manager to build partnerships, identify individuals in need, and connect them with services for suicide prevention and behavioral health support.

The role reports to the Program Director, requires a Bachelor’s degree and 3–5 years of outreach experience, and involves occasional evening/weekend hours, travel within the region, and collaboration with VA resources.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• 3 to 5 years of experience in outreach, preferably with Veterans or people with mental health, substance use, or IDD concerns.
• Excellent computer skills in Microsoft Office and general office software.
• Excellent written and verbal communication skills.
• Bilingual abilities are a plus.

Responsibilities

• Coordinate outreach efforts for the Veterans Program, including outreach plans for VA locations and shelters and establishing schedules with community providers.
• Screen program participants for eligibility using VA-approved forms and verify information.
• Provide or arrange wraparound services and connect clients to VA and mainstream resources.
• Communicate effectively with the service team while maintaining confidentiality to address issues.
• Attend supervision, meetings, trainings and participate in team planning; perform other duties as needed.
• Attend monthly community outreach meetings and assist neighbors experiencing homelessness with referrals and resources.
• Maintain computerized client records, logs and reports; may operate a van and conduct daily vehicle inspections.
