Job summary

Volunteers of America Chesapeake & Carolinas seeks a dedicated Community Outreach Case Manager in Fredericksburg, VA to support veterans through outreach, intake, and connection to behavioral health resources.

You will collaborate with VOACC teams to identify individuals in need, coordinate services, and ensure follow-through with community partners and VA resources. Evening hours and some weekend work may be required.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• 3 to 5 years of outreach experience with Veterans or individuals with mental health, substance use, or IDD concerns is preferred.
• Excellent computer skills in Microsoft Office and office products.
• Excellent written and verbal communication skills.
• Bilingual in any language is a plus

Responsibilities

• Coordinate outreach efforts for the Veterans Program, including developing an outreach plan and visiting VA facilities, shelters, and other veteran gathering locations.
• Screen potential program participants for eligibility using VA-approved forms and verify information with applicable agencies.
• Provide or arrange Suicide Prevention Grant Program services, including outreach, screening, education, emergency clinical services, case management, peer support, VA benefits assistance, and connection to resources.
• Maintain computerized client records, daily activity logs, mileage logs, and other reports as directed.
• Attend monthly community outreach meetings and collaborate with VOACC departments to improve referral pipelines.
• Travel throughout the community and to local sites; may drive a van/shuttle and perform related safety checks.
• Assist veterans experiencing homelessness in accessing resources and referrals.
• Other duties as assigned
