Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Community Outreach Case Manager to build partnerships and connect veterans and families with needed services in Virginia. The role requires evening/weekend hours, travel across the region, and collaboration with VOACC teams to strengthen referral pipelines for suicide prevention services.

You will report to the Program Director, maintain client records, and support homeless neighbors with access to resources while coordinating wraparound

Qualifications

• Bachelor’s degree in Human Services or related field.
• 3 to 5 years of outreach experience, preferably with veterans or mental health/substance use concerns.
• Knowledge of available community resources.
• Excellent written and verbal communication skills.

Responsibilities

• Coordinate outreach efforts for the Veterans Program and visit VA facilities, shelters and community sites.
• Screen participants for eligibility and verify information with applicable agencies.
• Provide outreach, mental health screening, education on suicide risk and prevention, and case management services.
• Assist with VA benefits and other government or community resources.
