Job summary

Volunteers of America Chesapeake & Carolinas seeks a Community Outreach Case Manager to build partnerships, identify individuals in need, and connect them with suicide prevention and behavioral health services in the Virginia region.

The role requires evening/weekend availability, strong communication, and ability to navigate VA-based resources while collaborating with the Program Director and other teams. Travel is required to engage community partners and veterans where they gather.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• 3–5 years of experience in outreach, preferably with Veterans or mental health concerns.
• Ability to explain an agency's purpose, programs and operations.
• Knowledge of available community resources and services.
• Self-starter with mission-driven approach; strong communication skills.

Responsibilities

• Coordinate outreach for the Veterans Program, visiting VA facilities, shelters and veteran hubs.
• Screen participants for program eligibility using VA-approved forms and verify information.
• Provide SSG Fox SPGP services such as outreach, mental health screening and family education.
• Advance referrals and coordinate VA benefits and other eligible resources.
• Maintain client records and reports; travel to community sites as needed.
