Job summary

Volunteers of America Chesapeake & Carolinas seeks a Community Outreach Case Manager to build partnerships, identify individuals in need of services, and support suicide prevention efforts. The role includes evening hours and potential weekend work, collaborating with VA and community providers.

Grant funded through December 2026 with potential extension; duties include outreach to veterans, eligibility screening, benefits assistance, wraparound services, and crisis support as needed.

Qualifications

• Bachelor’s degree in Human Services or related field is required.
• 3–5 years of outreach experience with Veterans or individuals with mental health, substance use, or IDD concerns.
• Ability to explain an agency’s purpose, programs and operations.
• Knowledge of available community resources.
• Self-starter who is mission driven and committed to helping people access our services.
• Excellent computer skills in Microsoft Office.
• Excellent written and verbal communication skills.
• Proven ability to resolve conflicts.
• Bilingual in any language is a plus.

Responsibilities

• Coordinate outreach efforts for the Veterans Program, including outreach plan development and visits to VA, shelters, and veteran hotspots.
• Screen potential participants for eligibility using VA-approved forms and verify information with applicable agencies.
• Provide outreach, mental health education, and case management services as outlined in program guidelines.
• Assist eligible individuals with VA benefits and other government benefits coordination.
• Travel throughout the community; maintain client records and logs; attend meetings and trainings.
