Volunteers of America Chesapeake & Carolinas seeks a Community Outreach Case Manager to build partnerships, identify individuals in need, and connect them with suicide prevention and behavioral health services.

The role requires evening/weekend hours, field visits, and collaboration with VA and local providers to coordinate resources for veterans and families in Virginia. A bachelor’s degree and outreach experience are required.

Formación

• Bachelor’s degree in Human Services or related field.
• 3–5 years of outreach experience with Veterans or people with mental health, substance use, or IDD concerns.
• Knowledge of available community resources.
• Self-starter who is mission driven and committed to helping people connect with our services.
• Excellent computer skills including Microsoft Office and related office products.

Responsabilidades

• Coordinate outreach efforts for the Veterans Program, visiting VA facilities, shelters and other locations.
• Screen potential participants for program eligibility using VA forms and verify information.
• Provide outreach, mental health screening, education, case management and peer support services.
• Assist with VA benefits and other entitlements, and coordination of health care, daily living and transportation needs.
• Maintain client records, logs and reports; drive a van/shuttle if needed; participate in meetings and trainings.
