Job summary

Volunteers of America Chesapeake & Carolinas seeks a Community Outreach Case Manager in Virginia to build partnerships, identify individuals in need of services, and improve referral pathways for VOACC’s Suicide Prevention initiatives.

The role requires a conventional work week with evening hours and possible weekend work, coordinating with veterans programs and community partners to connect neighbors with housing, health, and supportive services across the region.

Qualifications

• Bachelor’s degree in Human Services or related field.
• 3 to 5 years of outreach experience, preferably with Veterans or mental health contexts.
• Good communication skills and ability to work with diverse communities.

Responsibilities

• Coordinate outreach efforts for VOACC's programs, including establishing outreach plans and visiting VA facilities and community sites.
• Screen and refer individuals to VOACC services, verify eligibility, and follow up with applicable agencies.
• Provide or arrange support services such as case management, peer support, VA benefits assistance, and coordination of other benefits.
