Job summary

Volunteers of America Chesapeake & Carolinas is seeking a Community Outreach Case Manager in Burke, VA. The role focuses on building partnerships, identifying individuals in need, and coordinating suicide prevention and mental health services within VOACC’s ecosystem.

The ideal candidate has a Bachelor's degree in Human Services, 3–5 years of outreach experience with veterans or mental health concerns, and strong communication skills. Travel and evening/weekend hours may be required.

Qualifications

• Bachelor's degree in Human Services or related field is required.
• 3–5 years of outreach experience with Veterans or people with mental health, substance use, or IDD concerns.
• Excellent written and verbal communication skills; ability to work with diverse populations.

Responsibilities

• Coordinate outreach efforts for the Veterans Program, visiting VA facilities, shelters, and community sites.
• Screen participants for program eligibility using VA-approved forms and verify information with applicable agencies.
• Provide and arrange wraparound services and connections to VA and mainstream resources; ensure timely referrals.
