Volunteers of America Mid-States is hiring an Income Maximization Case Manager in Owensboro, KY. This full-time, hourly, non-exempt role supports Veterans by connecting them to income benefits, training, and services to end homelessness.

The role requires a Bachelor’s degree in social work or related field (or equivalent experience) and travel up to 70% across served counties. Training in case management and SOAR is required within 90 days of hire.

Formación

• Bachelor’s degree in social work or related field; less than five (5) years of experience preferred.
• Five (5) years of experience in the field with no degree is acceptable.
• Veteran with three (3) years of related work experience is eligible.

Responsabilidades

• Income Maximization Case Manager connects Veterans with benefits, including SOAR and other mainstream sources.
• Perform assessment and develop/monitor case plans with follow-up activities.
• Establish linkages with agencies and service providers in the area/community.
• Provide referrals and coordinate with VAMCs and other providers.
• Connect Veterans with American Job Centers and career centers.
• Decide how resources are allocated to participants based on need.
• Educate participants on available services and participant rights.
• Provide referrals for financial planning, transportation, income support, fiduciary and legal services, childcare, housing counseling.
• Complete required documentation within 48 hours and enter data into HMIS.
• Demonstrate good clinical judgment and cultural sensitivity in decision making.
• Collaborate with PQI and other SSVF staff; serve as liaison with HUD-VASH teams.
