Job summary

Volunteers of America Mid-States in Bowling Green, KY is seeking an Income Maximization Case Manager for the Supportive Services for Veteran Families (SSVF) program. This full-time, non-exempt role focuses on helping Veterans end housing crises by connecting them with income sources, benefits, and employment opportunities.

Qualifications

• Bachelor's degree in social work or related field, or 5 years in the field without a degree.
• Veteran with 3 years of related work experience also acceptable.
• Ability to assess family needs and access local resources.
• Commitment to project principles; travel up to 70% of the time; driving license.

Responsibilities

• Connect Veterans with benefits including SOAR applications and other programs.
• Perform assessment and develop/monitor case plans with follow-up.
• Establish linkages with agencies and service providers in the area.
• Provide referrals and coordinate with VAMCs and other providers.
• Assist Veterans in accessing employment education and career centers.
• Document progress within HMIS within 48 hours of contact.
