Job summary

VOA Mid-States is seeking an Income Maximization Case Manager in Owensboro, KY to assist Veterans who are homeless or at risk of homelessness. The role focuses on securing income through benefits, training, and employment while guiding participants toward housing stability and self-sufficiency.

Responsibilities include assessing needs, coordinating with partners, facilitating access to resources, and documenting progress in HMIS.

Qualifications

• Bachelor’s degree in social work or related field required.
• Less than five years of experience; or a veteran with three years of related work experience.
• Must be willing and able to travel between the counties served up to 70% of the time.
• Must complete required case management training within 90 days and SOAR online training.

Responsibilities

• Income Maximization Case Manager connects Veterans with benefits, including SOAR and other programs.
• Perform assessments and develop/monitor case plans with follow-up activities.
• Establish linkages with local agencies and service providers.
• Provide referrals and coordinate with VAMCs and other providers.
• Connect Veterans with American Job Centers and career centers.
• Decide how resources are allocated based on need.
• Educate participants on services availability and rights.
• Provide referrals for supportive services (financial planning, transportation, etc.).
• Complete required documentation within 48 hours and enter HMIS data.
• Demonstrate good clinical judgment and cultural sensitivity in decision making.
• Collaborate with other SSVF staff and participate in case consultations.
