Job summary

Volunteers of America Mid-States is seeking a dedicated Case Manager I to support Veterans through the SSVF program in Louisville, KY. The role focuses on assessing needs, developing case plans, connecting to community resources, and helping veterans achieve housing stability and self-sufficiency.

A bachelor’s degree in social work or related field (or equivalent experience for veterans) is required, with strong communication, organization, and teamwork skills; travel up to 70% is expected

Qualifications

• Bachelor’s degree in social work or related field required or equivalent experience for veterans.
• Excellent verbal and written communication skills and strong organizational abilities.
• Willingness to travel up to 70% within service counties and have reliable transportation.

Responsibilities

• Provide outreach to veterans and families to increase program participation.
• Perform assessment and develop/monitor case plans with follow-up.
• Coordinate with HUD-VASH, SSVF teams and local service providers to connect participants with housing and supportive services.
• Assist participants in obtaining VA benefits and other public benefits.
• Document progress and case notes within required timelines and HMIS.
