Job summary

Veteran Pathways of New England in Devens, MA is seeking a Case Manager to develop stabilization plans addressing housing, employment, and wellness for veterans and service members. The role combines office-based work with field visits across the Massachusetts service area.

You will maintain a full caseload, ensure timely, accurate documentation, and collaborate with outreach staff to identify and assist Veterans in need. Travel within the service area is required.

Qualifications

• Associate or Bachelor’s degree preferred; equivalent directly related experience may be considered.
• 1-3 years of experience in case management or related human services field.
• Strong organizational, communication, and problem-solving skills.
• Reliable transportation and ability to travel within MA for field work.

Responsibilities

• Provide case management to support housing stability and plan development.
• Perform intake assessments and manage a full caseload with documentation.
• Coordinate with VA, state and local benefits; connect clients to resources.
• Engage communities and conduct outreach to Veterans and service members.
