Job summary

SMOC is seeking a Case Manager in Massachusetts to support veterans and at-risk individuals by developing service and rehousing plans, coordinating with care teams, and assisting with health, budgeting, and daily living needs. You will conduct intake, monitor progress, document meetings, and advocate to connect clients with housing resources and services.

Qualifications

• Bachelor's Degree or minimum three years in Human Services.
• Sensitivity to low-income families from diverse backgrounds.
• Ability to work independently and manage time effectively.
• Strong written communication skills.
• Valid driver's license and insurance compliance.
• Assessment, advocacy and case management skills.
• Bilingual preferred.

Responsibilities

• Work 1:1 with new placements to ensure housing readiness and orientation.
• Complete intake and needs assessment covering income, budgeting, health and daily living.
• Meet weekly to monitor re-housing plans and document progress.
• Document all client meetings and safety inspections of units.
• Coordinate with care teams to address barriers and facilitate services.
• Schedule flexible weekly meetings to accommodate work schedules.
• Develop and implement housing action plans and rehousing strategies.
• Attend team meetings and maintain confidentiality according to laws.
